Administrative Forms
Visit the CVHS Athletic Department for a list of forms required to participate in the Crescent Valley High School Wrestling Program.
These forms include the:
Physical exam form (required for freshmen, juniors, new students, and first-year athletes)
Emergency Contact Card
Athletic Code of Conduct which must be completed and signed before participation in wrestling for the coming season.
Forms and fees ($150) must be submitted by the first day of practice.
